I suggest using the test procedure that I posted the link to above, to test all four units.

I don't have a suitable lamp so for now I just followed the shop manual.
What would the lamp test show that the shop manual procedure doesn't?

Maybe replace the whole loom with the NOS I got with the bike?

Completed the carb swap between left and center and moved throttles, needles etc. Cleaned choke plungers and springs on both carbs. No difference.

Measured resistance on the pickup coils. All OK. Measured low and high speed coils. High speed seems OK with 5-5.2 ohms. Low speed measures 209 ohms however. A bit high but maybe not enough to cause trouble?

Hi/Lo Coils are ok... if they were bad it would affect all cyls the same.
Best bet now is with the CDI boxes. The A or B box could be causing the problem.
Are you sure the sparkplug lead isn't arcing to the cyl?

I swapped 2 of the 3 CDI units and got spark, but maybe the spark is weak?

Basically I just followed the 3 steps on page 117 in the Kawasaki H1 shop manual. Got spark every time but don't know how good it is. Measuring the components could maybe tell me?

Not sure the spark plug lead is good really. Is arching visible in a dark room?

Yes, a dark room is a good way to find arcing leads or you could force them away from metal temporarily with cardboard boxes, etc.
Where you have a firing cyl at higher rpms it may not show up simply by taking resistance measurements on CDI... you have a borderline condition rather than a catastrophic. Actually swapping CDI boxes rather than temporarily just looking for spark would be proof. A weak spark in air may be zero spark under compression. Likewise, you could swap inputs & outputs on the CDI rectifier box.
Again, the most likely cause of the problem you are facing is the pickup coil. Sometimes the solder joint on the pickup is poor and just moving the lead will make a difference in resistance measured.
